WebbGender: Male. Gross oedematous swelling and increased blood flow in the scrotum overlying both testes with extension into the groins. Underlying testes show minor reactive hyperaemia but there is no hydrocoele. No obvious gas in the soft tissues to indicate necrotising fasciitis. No focal fluid collection to indicate abscess formation. Culture was positive in 31% (22/70) either from blood or from skin aspirates (10 from blood, 9 from skin, and 3 from both). 73% of culture (16/22) showed growth of gram negative organisms.
